St. Luke’s leverages Get Real Health’s InstantPHR™ solution and health information exchange from Caradigm to improve the patient experience and work toward meeting Federal Meaningful Use Stage 2 patient engagement requirements
For Phase 1 of the project, Get Real Health’s award-winning InstantPHR™ patient engagement solution will provide St. Luke’s facilities with an integrated patient portal website that will help improve the patient experience, make patients’ medical information more accessible, and help St. Luke’s facilities work toward meeting the patient engagement requirements of the Medicare and Medicaid EHR Incentive Programs, also called “Meaningful Use – Stage 2.”
Caradigm’s health information exchange (HIE) software integrates various data sources throughout the system to create an enterprise-wide HIE for St. Luke’s. The InstantPHR-powered patient portal website, called eVantageHealth, has been connected directly to the HIE (rather than tethered to individual EHR systems). The result is an innovative solution that offers a truly unified and holistic view of St. Luke’s patients’ medical information to patients, their healthcare providers, and other authorized third parties — like out-of-network providers and family caregivers. Using the portal, patients will be able to view their health records, download them to their computer, and transmit them to authorized third parties.

St. Luke’s InstantPHR solution includes:
- A patient-centric application that enables patients, family members, and providers to collaboratively manage healthcare and improve overall health outcomes and quality of life.
- Spanish-language support for the patient-facing widgets, which can help meet the needs of St. Luke’s Spanish-speaking community.
- Coordination of care between patient and providers, as well as among providers through secure messaging and the ability to access patients’ complete medical information, from across the entire St. Luke’s network of facilities and providers.
- Patient data will be rich and robust as a result of integration with St. Luke’s HIE.
St. Luke’s is making history with this approach to patient engagement. Patients will be able to leave a Network healthcare appointment with their own medical records available from their computer, tablet or smartphone. Patients and St. Luke’s doctors will also be able to securely message one another and share information through the system, thus enhancing their mutual understanding of test results, discharge instructions and related medical information. This new system makes the patient part of the process and an equal participant in managing his or her own care.
Patient Enrollment Support
St. Luke’s is also procuring patient-facing marketing materials from Get Real Health to use in its patient outreach. St. Luke’s will adapt the engagement tools, which include brochures and a video, to help patients understand and enroll in the system.
