At this week’s Health Datapalooza IV, Get Real Health was invited to present InstantPHR, their award-winning connected care platform, which is successfully being used in the United States and abroad to improve patient outcomes and increase efficiencies in healthcare delivery. More than 2,000 of the most influential health information technology (HIT) leaders flooded Washington, D.C. for the conference, which was created to carry out the government’s mission to encourage greater access to personal health data.

The InstantPHR platform includes a patient portal, healthcare provider dashboard, and an administrative console along with nearly 200 widgets. These include robust data visualization tools, self-management action plans, alerts, reminders, journaling tools, surveys, and encrypted messaging.
