This project was Piloted within the in the Netherlands with the Tergooi hospital system.
Challenge
The challenge in this instance was to ensure that palliative care populations, associated care teams and families would have access to a suite of tools to ensure care was being provided as effectively as possible while keeping all parties in sync with updates. Most patients had multiple care teams preventing any one provider from obtaining a global view of overall patient care.
Solution
Providers from various care teams associated with the patient had the ability to group, share and update patient information based on new data while family members granted access have up-to- date information with respect to their loved one(s).
Benefits
Benefits included the grouping of patients according to care groups for care team access, mobile access to information for all, custom survey functionality specific to palliative care, widget specific configurations to better serve specific patient populations with data most relative to their needs.
Relevance
This is the first project for Get Real Health in the Netherlands and the first time Get Real Health focused primarily on serving a palliative care population. From a technical perspective this also the first implementation of an EMR launch sequence being used to leverage a Smart on FHIR app with CHBASE to deliver users with the ability to communicate with providers via teleconference.
